About the Opportunity

Sunrise Health Service (SHS) now has a rewarding opportunity for a full-time Program Coordinator (Chronic Disease) to join their multidisciplinary Population Health team on a 2 year contract, with the possibility of extension. The team is partially funded through the Integrated Chronic Disease (ITC) Program via NT PHN, supported through the Aboriginal Medical Services Alliance Northern Territory.

Reporting to the Outreach Manager, your primary responsibility will be supporting Indigenous people with chronic conditions and complex care needs to access a range of culturally appropriate services in both clinical and community settings that are tailored to meet the needs of the individual patient.
