South West Healthcare is seeking a qualified Allied Health or Nursing Professional with significant clinical experience to oversee our Ambulatory Rehabilitation Services, Community Rehabilitation Programs, Specialist Clinics and Transition Care Program.

The role has a strategic and operational leadership focus, through the design and implementation of an innovative and person-centred model of care across the above programs. Priorities will include:

  • Enhancing access to services
  • Connecting Ambulatory Rehabilitation services between acute hospital and community based services to ensure connection of care throughout the patient journey
  • Where appropriate, focusing on integration and coordination of Ambulatory Rehabilitation Services
  • Measuring and improving clinical outcomes and patient experience

The successful applicant will have an engaging and collaborative leadership style as well as excellent communication and influencing skills. Applicants should have a strong track record in change management and leadership in a relevant clinical setting.

Remuneration, benefits/salary packaging and conditions of employment will be negotiated with the successful applicant commensurate with qualifications, skills and experience.
