Access and Support Worker

  • $83K - $91K + Super (Pro rata)
  • Temporary position to 30 March 2019
  • Part Time 24 hours per week

The Aged and Community Support Branch is seeking an experienced professional to work within an established team. The position involves supporting people from diverse backgrounds who, due to diversity characteristics, have barriers to services and require assistance to access Commonwealth Home Support Programme, HACC Program for Younger People and other services.

You will have extensive knowledge and experience in working with mature age residents of the Moreland municipality and people with a disability, providing information, support, advocacy and guidance that recognises and respects personal life choices and circumstances.

You will have excellent communication skills and are able to build rapport easily with people from a wide range of backgrounds. You will also have excellent interpersonal and problem solving skills with the ability to support and assist people to identify and access relevant and meaningful ongoing support services. 

You will be committed to building positive working relationships with colleagues both within the organisation and with external agencies.

The successful applicant will have an understanding of and commitment to an approach which is person centred and have qualifications and experience in health or community services, including advocacy, community outreach and case management.
