The City of Stonnington is seeking a diligent and professional Service Coordination Officer to join our Care Services Unit. This is an excellent opportunity to further develop your staff management skills and positively contribute to our community through developing service plans to assist our residents aged over 65 years.
As the Service Coordination Officer you will be responsible for supervising a small Service Coordination team, monitoring and reviewing residents’ eligibility for the Commonwealth Home Support Program and providing support to clients. Additionally this position will be responsible for developing and monitoring of client service plans, identifying and developing appropriate referral pathways, conducting risk assessments for clients within their residence and developing risk management strategies as required.
To be successful in this position, you will have a tertiary qualification in the relevant field, experience in an Aged and Disability Services environment, an understanding of issues related to older people, people with disabilities and their carers, the ability to manage and supervise staff effectively and knowledge of Carelink+ (client management data systems), My Aged Care or other relevant computer systems.
