About the role

The role of the Diabetes Nurse Educator is to provide direct clinical care and intervention. They will apply knowledge of current and interdisciplinary best practices in diabetes management to guide client-centred clinical decision making, referrals, and care.

This position is funded by Community Health and is based within the Health Impacts North Multidisciplinary Team.

Your daily duties will vary but will include:

  • Accountable for the day-to-day Diabetes Education service ensuring the provision of high-quality care for a diverse range of population groups with a wide array of acute and chronic conditions;
  • Liaise with Program Managers to ensure streamline office systems and referral pathways are in place to support best practice service delivery;
  • Conduct education sessions to primary care and community health service providers to promote integrated and best practice services;
  • Conduct group education sessions with clients and their carers that reflect community need;
  • Plan, implement and evaluate programs in response to community needs and in consultation with other staff;
  • Contribute to and participate in health promotion programs as requested;
  • Perform other duties consistent with the aims and objectives of cohealth as directed by the Program Manager

Who are we looking for?

  • Qualified nurse;
  • Knowledge of current diabetes medications and management;
  • Confident in use of patient management software systems.
