headspace is an initiative funded by the Australian Government (Department of Health) which provides holistic support for young people aged 12 – 25 and their families. headspace aims to improve young people’s mental and physical health, address alcohol and other drug issues, and assist with access to vocational and employment services. Black Swan Health Ltd is the lead agency for headspace Osborne Park.

As the headspace Intake Coordinator, you will be part of a dedicated multidisciplinary team offering a high quality, early intervention response to young people and their families. You will have the chance to expand your experience working with young people by having a direct role in the assessment and coordination of their care as well as offering ongoing counselling. This is a rare opportunity to join headspace in an intake coordinator capacity.

Responsibilities 

  • Co-ordinate the intake process including management and triage of the intake register, appropriate waiting list allocation of referrals and following up with referrers.  
  • Provide initial information, support and psycho educational content to young people and families accessing the service via telephone. 
  • Provide initial engagement, screening and bio-psychosocial (HEADSS) assessment of young people, particularly those who are deemed a priority, including referral onto appropriate services both internal and external to headspace Osborne Park, in collaboration with the headspace clinical team. 
  • Provide on-going therapeutic support to young people.
  • Establish, foster and maintain close referral pathways with relevant mental health, carer organisations and other relevant service providers in the local area.  
  • Monitor complex and higher risk client referrals and ensure that appropriate priority assessment, management and follow up support and/or referral are provided in collaboration with the Clinical Lead.  
  • Chair the regular Intake & Allocation Meetings and facilitate resulting actions.

Essential Competencies 

  1. Approved tertiary qualifications in Psychology, Social Work, Nursing, OT and eligibility for registration as required e.g. AHPRA, AASW 
  2. Demonstrated experience working in a multi-disciplinary case management environment with a mental health client group/ or vulnerable youth services with at least 3 years’ experience. 
  3. Proven ability to develop rapport and effective professional relationships with young people, key stakeholders and associated organisations. 
  4. Demonstrated working knowledge and experience using contemporary models and methods of supporting young people living with mental illness, specifically risk assessment and management. 
  5. Commitment to a social inclusion framework of working within the community. 
  6. Demonstrated skills and/or knowledge and experience working with young people presenting with co-occurring mental health and alcohol and other drugs issues; young people who are homeless or at risk of becoming homeless; people identifying as lesbian, gay, bisexual, trans, intersex, or questioning; and young people with other barriers to accessing mental health support.
