Clinical Services Manager- Aged Care

Your Key Responsibilities:

  • Monitor and review ongoing customer needs, physical, and emotional well-being, making appropriate recommendations for supports or services.
  • Actively respond to new client inquiries, manage referrals, upgrades, and update client information.
  • Analyse client issues, problem-solve to achieve realistic and practicable client-centered solutions.
  • Respond to client clinical care requirements and health and well-being feedback, managing client issues and outcomes.
  • Keep up to date with developments, legislation, and regulations relevant to the areas of responsibility.
  • Support customers to achieve progress towards documented person-centered goal-directed outcomes.
  • Provide case management and ensure the day-to-day support and well-being of clients through casework, advocacy, and planning.
  • Create Care Plans and maintain budgets for existing and prospective HCP clients to ensure that their care needs are met within funding approval.
  • Prepare client agreements and provide a clear explanation of the agreement while acting as a client advocate.

Your Skills & Experience:

  • Tertiary qualification in a relevant field e.g., Case Management, Social Work, Health, Allied Health, Nursing.
  • AHPRA registered is essential.
  • Broad knowledge and experience in the aged care sector.
  • Previous experience in the Aged Care industry is ideal.
  • High level of written and verbal communication, administrative, and organisational skills.
  • Customer-focused with excellent communication and problem-solving skills.
  • Ability to take initiative, pursue opportunities, and optimise resources.
  • Flexibility, adaptability, and analytical thinking.
  • Current driver’s license and police check.
  • Good computer skills and exposure to client management systems.
