Four staff members at Adelaide’s Lyell McEwin Hospital have been “counselled” after removing a radiologist's comments from a patient's medical record.

The staff reportedly deleted comments from a patient's record related to the ordering of tests and delays in reporting test results.

Authorities have completed a review of the situation, and say that four people in middle management and above were involved in changing the record.

It is understood that the details were not destroyed, but rather separated.

The move did made the records invisible, so they could not be seen.

Health Minister Jack Snelling said Crown Law is considering forwarding the matter to the Department of Public Prosecutions.

The Opposition has used the opportunity to take shots at the SA government, saying medical staff struggle with several computer programs in use.

The Lyell McEwin Hospital introduced a new computer medical imaging system earlier this year.
